Pain on the outside of the knee, often associated with running or repetitive lower body activity

What it is

IT band syndrome typically presents as pain on the outside of the knee, especially during running or repetitive movement.

A lot of people think the IT band itself is the problem, but the issue is usually how the surrounding muscles and joints are working together.

In some cases, there may be irritation where the IT band interacts with the outside of the knee—but more often, the bigger issue is how load is being managed through the hip and leg.

Why you’re experiencing it

This is almost always a buildup of repetitive stress.

Common contributing factors include:

  • Weakness in the glutes or hip stabilizers
  • Poor control during single-leg movements
  • Limited hip mobility
  • Running mechanics or training errors
  • Sudden increases in mileage or intensity

When the hip isn’t doing its job well, more stress gets passed down to the outside of the knee.

How it shows up

  • Pain on the outside of the knee during running
  • Pain that worsens the longer you go
  • Discomfort with downhill running
  • Tightness along the outside of the leg
  • Pain that eases with rest but returns quickly
