Shoulder pain with lifting, reaching, or overhead activity
What it is
Rotator cuff pain typically presents as discomfort with lifting your arm, reaching overhead, or performing pressing movements.
A lot of people assume something is torn, but that’s not always the case.
More often, it’s related to how the shoulder is moving and being loaded.
In some cases, there may be irritation or strain of the rotator cuff tendons—but more often, the bigger issue is how the shoulder is functioning within the system of the upper back, shoulder blade, and ribcage.
Why you’re experiencing it
This usually develops over time with repeated stress.
Common contributing factors include:
- Limited shoulder blade or thoracic mobility
- Poor control of the shoulder blade
- Weakness in stabilizing muscles
- Overuse from lifting or repetitive activity
- Poor load management or programming
When the shoulder isn’t moving efficiently, the rotator cuff ends up taking on more stress than it should.

You might notice:
- Pain with overhead movements
- Discomfort when lifting or pressing
- Pain reaching behind your back
- Weakness or instability
- Pain during or after workouts
